Scan or take a photo of the completed form in order to email it back to us as shown in step 3 below. 

This form contains important preparation and care information for your tree. 

Please note: To provide you with the best value and to help ensure that your tree establishes itself for a long healthy life, we are giving away small container trees, most ranging from 2 to 5 gallons.
